We are seeking a skilled Registered Nurse Case Manager to join our durable medical services team. This role is crucial in assessing patient needs, coordinating care, and ensuring effective communication among all parties involved in patient care. 

Responsibilities

As a Registered Nurse Case Manager, you will: 

  • Coordinate DME Services: Oversee the coordination of durable medical equipment, supplies, and related services by collaborating with physicians, vendors, therapists, and internal departments to support patient care needs.  
  • Obtain and Manage Physician Orders: Communicate with MD offices and healthcare providers to obtain signed orders, letters of medical necessity, supporting documentation, and any additional clinical information required for equipment and supply processing and approval.  
  • Review Clinical Documentation: Assess patient documentation to ensure medical necessity criteria are met for equipment requests, authorizations, and continued services. 
  • Track Authorizations and Follow-Up: Monitor pending orders, approvals, renewals, and physician signatures to ensure timely processing and uninterrupted patient support. 
  • Ensure Confidentiality: Maintain confidentiality of all client and office records in accordance with HIPAA guidelines and the Privacy Act PHI. 
  • Provide Patient Support and Education: Ensure effective communication and collaboration among the care team to provide cohesive and comprehensive care. Assist patients and families with understanding equipment use, service coordination, and the DME process while promoting a positive patient experience. 
  • Additional Duties: Perform other duties as assigned to support the team and enhance patient care. 

Qualifications

What We’re Looking For 

  • Current RN License: Valid and active Registered Nurse license in good standing. 
  • Experience: At least one year of experience in a durable medical equipment, care coordination, case management, or related healthcare settings is preferred. 
  • Organizational Skills: Strong attention to detail with the ability to manage multiple patients, follow-ups, and deadlines efficiently.  
  • Tech Savvy: Proficient in computer and internet-based applications, EMR systems, as well as office equipment. 
  • Communication Skills: Excellent verbal and written communication skills with the ability to professionally and compassionately interact with patients, family members, internal partners, vendors, and physicians. 
  • Knowledge of Medical Equipment: Working knowledge or durable medical equipment and related patient care needs.  
  • Background Check: Must pass a criminal background check and sanction screening. 
  • Professionalism: Professional demeanor and appearance, strong customer service skills, and ability to work collaboratively within a team environment are necessary. 

Pay Range

USD $40.00 - USD $45.00 /Hr.
