We are seeking a dedicated Registered Nurse Case Manager / Intake Nurse to join our care team. In this dual role, you will be the first point of contact for new patients, guiding them through the intake process, and managing ongoing care to ensure quality outcomes and coordination across services.
Patient Intake & Assessment
Conduct comprehensive assessments for new patients to determine care eligibility and needs under EEOICPA.
Review medical records, physician reports, and supporting documentation to ensure accuracy and completeness.
Educate patients and families on available services and the care process.
Case Management
Coordinate nursing care, home health aide services, and therapies once authorized by the Department of Labor.
Develop and monitor individualized care plans to ensure patients receive the highest quality of care.
Maintain open and timely communication between patients, families, providers, and internal care teams.
Evaluate patient progress and adjust care plans as needed.
Compliance & Documentation
Ensure confidentiality of patient information in accordance with HIPAA and Privacy Act PHI regulations.
Accurately document all assessments, communications, and care activities in the patient record.
Adhere to all organizational and federal program standards.
Team Collaboration
Partner closely with Case Managers, Field Nurses, and Administrative Teams to streamline care coordination.
Participate in interdisciplinary case reviews and meetings.
Provide support and education to team members and patients as needed.
License: Current and active Registered Nurse (RN) license in good standing.
Experience: Minimum of one year of experience in home health, case management, or intake nursing.
Skills:
Strong assessment and clinical decision-making skills.
Excellent communication, organizational, and time management abilities.
Proficient in computer systems and electronic medical records (EMR).
Working knowledge of patient-care equipment.
Other Requirements:
Must pass a criminal background check and sanction screening.
Professional demeanor and appearance.
Valid driver’s license, reliable transportation and willingness to travel for home visits.
